Summary:•Central nervous system aspergillosis has a poor prognosis.•In immunocompetent patients treated with dual antifungal drugs and surgical resection, mortality can be as high as 66%.•A new voriconazole monotherapy can successfully treat invasive orbitofrontal aspergillosis without surgical resection. In this case report we present an immunocompetent 64-year-old patient presenting with an orbitofrontal invasive aspergillosis treated successfully with voriconazole monotherapy following biopsy and orbital decompression.
