Summary: | Isometric ligament reconstruction with two graft limbs is used for chronic ligament injuries. Our technique facilitates proper tensioning. It is described in thumb metacarpophalangeal joint ulnar collateral ligament reconstruction. After passing through a tunnel in the base of the phalanx, the two limbs of the graft are docked inside the metacarpal socket. The graft is then fixed under continuous manual tensioning using a Pushlock® anchor. This technique is easy and reproducible; introduction inside the tunnel does not compromise the graft and tensioning is under continuous control. This technique is applicable to any hand, wrist, or elbow ligament reconstruction.
Le réglage tensionnel des reconstructions ligamentaires est crucial. L’intérêt d’un tunnel borgne proximal avec fixation isométrique par vis d’interférence est bien démontré, mais le réglage est difficile car nécessite de prévoir la longueur de la greffe. L’introduction des extrémités dans le tunnel abîme la greffe malgré le faufilage. L’utilisation d’une boucle de nylon à l’extrémité d’une ancre Pushlock® amène les greffons tout au fond du tunnel au niveau de boucles et non d’extrémités ; la tension est réglable à tout moment sans aucune mesure. Cette technique simple est applicable pour toute reconstruction ligamentaire de la main, du poignet ou du coude. |
