Summary: | To the Editor: The accompanying article by Abdelbaser and colleagues in this edition of The Journal of Clinical Anesthesia compares the efficacy of inhaled levosimendan to intravenous levosimendan in reduction of elevated pulmonary artery pressure (PAP) in children that underwent cardiac surgery for repair of left-to-right shunts. [...]the population being studied and the indication for administration should be closely examined going forward. In Abdelbaser's study, all patients with heart failure were excluded, so that the effects of levosimendan could be isolated to just the effects on pulmonary vasculature, as opposed to the mixed picture of increased inotropy and vascular smooth muscle relaxation. |
