Summary: | •The incidence of anal cancer is rising in the United States, and most anal squamous cell carcinoma (SCCA) is associated with high-risk human papillomavirus subtypes.•Not all patients with SCCA have the typical risk factors for development of the disease.•SCCA rarely metastasizes, but most common sites are liver, lung, extrapelvic lymph nodes, peritoneum, and bone.•This is a case of a patient with SCCA that metastasized to his sigmoid colon without evidence of other distant disease.•Pembrolizumab is an option for patients with advanced SCCA that is not amenable to chemotherapy or surgical resection. It has also been recently approved for tumors with high tumor mutation burden. |
