Summary:Highlights•The complexity of the illness course and diverse clinical features of bipolar disorder (BPD) make the diagnosis challenging. Early identification of risk factors of BPD is important for early diagnosis and intervention. •The clinical phenomenology and characteristics of prodromal symptoms in Chinese patients with bipolar disorder (BPD) prior to their index mood episode were examined in this study. •Characteristic affective and psychotic symptoms, including depressed mood and subthreshold hypo (manic) symptoms occurred in the prodromal phase. The pre-depressive prodromal symptoms lasted longer than the pre-(hypo) manic prodromal symptoms. •Identifying prodromal symptoms of BPD may be beneficial for early diagnosis and intervention before patients develop full manic/depressive episodes.
