Summary:Objective To compare features of pre-menarchal and post-menarchal patients with adnexal torsion. Methods We reviewed hospital records to note examination findings, laboratory work-up, imaging results, operative findings and course during hospital stay for 56 girls aged from 7 days to 17 years with adnexal torsion presenting between January, 2012 and December, 2020. Results 31 girls were pre-menarchal. Pain was the most common symptom. There were significant differences in the volume of the ovary visualized in ultrasound in amenorrheic and menstruating girls [median (IQR) 78234 (39600, 183600) mm 3 vs 243432 (158661, 388800) mm 3 ; P =0.004]. Pain was the most common symptom. Over the years, there was an increase in laparoscopic procedures, and efforts to preserve the ovary after the torsion. Conclusions The differential diagnosis in the case of abdominal pain should include adnexal torsion both in non-menstruating and menstruating girls.
