Summary:OBJECTIVES. Do we need different hip systems for Primary and Revision TH-Replacement or can one highly modular system replace them successfully? METHODS. Analysis was done using one femur stem system containing monobloc to sophisticated modular implants. Different implant combination are possible: metaphysial or diaphysial anchorage, static or dynamic distal interlocking, step less ante version, hydroxyapatit coating etc. In a prospective study 226 patients were evaluated. RESULTS. Revisions 32%, femoral neck fractures 21%, intertrochanteric fractures 20%, pathologic fract. 15%, Coxarthrosis 9%, others 3%. Distal interlocking statically n=43, dynamically n=13. Mean follow up 11.4 months. RESULTS. Example: intertroch. Fractures with coxarthrosis n=45, 8 deceased, 23 = 62.2% examined. Bony consolidation in all cases, one subsidence due to marked medial comminution, walking ability 97%. The mixed price calculation for a THR system containing a monobloc and highly modular implant shows a significant cost reduction. The quality of surgery improves with the number of cases done by one team. This can be demonstrated by a significantly shorted operation time = 150 min down to 75 min. CONCLUSIONS. One THP systems with sophisticated modularity allow economical improvements as well a quality enhancement. A 'baseline'technology as used by car manufactures brings predictable advantages to endoprosthesis technology.
