Summary:An intraoperative frozen section of the left adrenal mass was obtained, confirming necrotizing granulomatous reaction and numerous budding yeasts consistent with Cryptococcus spp (Figure 2). Discussion Fungal infections of the adrenal glands are a rare cause of primary AI in the United States. 1 Cryptococcal infection of the adrenal glands is a rare and opportunistic infection with only a few case reports identified in the literature and even fewer cases described in the immunocompetent patient population. 3 This case highlights the importance of considering infections other than Mycobacterium tuberculosis and Histoplasma capsulatum as the possible etiology of primary AI. Cryptococcus neoformans is naturally acquired through inhalation of spores found worldwide in soil, decaying wood, or bird droppings and leads more frequently to primary pulmonary infection in immunosuppressed patients.
