Summary:Abstract Left ventricular outflow tract obstruction is a rare complication following transcatheter mitral valve implantation. Diagnosing the underlying cause is mandatory to select from different treatment options. We report a case of stent-graft implantation into the left ventricular outflow tract for dynamic left ventricular outflow tract obstruction caused by systolic anterior motion of the anterior mitral valve leaflet (SAM). Acute haemodynamic deterioration after transcatheter mitral valve implantation (TMVI) can be caused by left ventricular outflow tract obstruction (LVOTO). Graphical Abstract
