Summary:Allergic bronchopulmonary aspergillosis (ABPA) is a disease affecting asthmatic subjects with variability in its clinical presentation. Symptoms typically consist of cough producing golden-brown mucous plugs and occasionally include fever or malaise, although patients may be asymptomatic. Radiologic studies commonly reveal pulmonary infiltrates or central bronchiectasis, and peripheral blood eosinophilia may be present in the absence of systemic corticosteroid use. Confirmation of the diagnosis requires identifying immediate cutaneous reactivity to Aspergillus spp and serologic evidence of elevation of total serum IgE (>1000 ng/mL), elevation of serum IgE and IgG antibodies to Aspergillus fumigatus, and the presence of precipitating antibodies to A fumigatus. We report the case of a patient who was first seen with bilateral lung abscesses and underlying ABPA.
