Summary:HAT is a major complication of OLT, occurring in approximately 10% of adult recipients. Up to 50% of patients with HAT end up with complications, including biliary strictures, biloma, and abscess. HAT occurring early after liver transplantation is particularly dangerous, associated with acute fulminant hepatic failure, biliary tract necrosis, and relapsing bacteremia. Accurate and prompt diagnosis of early HAT is important, as recent evidence has shown that emergent revascularization leads to improved graff salvage and patient survival with a relatively low rate of late biliary complications. The Doppler ultrasonographic diagnosis of HAT is based on indirect findings, failure to identify or obtain a Doppler signal from the hepatic artery. In this report we describe direct sonographic visualization of thrombus within the hepatic artery.
