Summary:Abstract Transmission of cytomegalovirus (CMV) is uncommon in patients transplanted with CMV-mismatched pancreatic islets, and CMV-seropositive recipients rarely experience reactivation of the virus or reinfection from a CMV-positive graft. This study describes three cases of CMV infection following islet transplantation for type 1 diabetes despite prophylaxis with valganciclovir. Further studies are needed to evaluate risk factors for CMV transmission and reactivation in this patient population.
