Summary: | The Lusaka District programme is among the largest and most successful nevirapine-based MTCT-prevention services in the world;10 nearly 3000 pregnant women are tested for HIV-1 infection each month. However, the history of the programme's implementation shows how offering nevirapine to women of unknown HIV-1-status could greatly improve population coverage of the drug. The programme began in two clinics with intensive training in all practical aspects of MTCT prevention. The majority of time was spent on VCT training of midwives and other health-care workers. During the 19 months that it took to fully deploy MTCT-prevention services in Lusaka, more than 7000 HIV-1-infected women gave birth in clinics that did not yet have VCT services; thus, more than 7000 infants were exposed to HIV-1 but received no prophylaxis, despite the district pharmacy being fully stocked with nevirapine. |
