Summary:A 65-year-old right-handed man experienced the sudden onset of sensory aphasia and weakness of the right side of his body. He was hospitalized 2 days later, and a computed tomographic scan revealed subcortical hemorrhage in the left parietotemporal cortex. Thirty minutes after admission, he had a focal seizure beginning on the right side of the face and spreading to the right arm and right leg, without loss of conciousness. Ictal Tc-99m HMPAO SPECT showed hyperemia surrounding the hematoma. Furthermore, this hyperperfusion extended to the ipsilateral basal ganglia and occipital lobe. In the 5 minutes after the first SPECT, the convulsions gradually dissipated. On a second SPECT obtained 7 days after admission, the hyperperfusion had disappeared.
