Summary:The purpose of this study was to describe attitudes and behaviours of pharmacy staff toward near misses through a survey and interviews. Themes of an acknowledgment of pharmacy process changes resulting from near miss reporting, feelings of personal responsibility, minimizing the importance of reporting near misses, and a need for further education were evident in both the survey and qualitative discussions. This survey is useful to inform educational efforts and to encourage near miss reporting.
