Summary:Although the majority of Sertoli cell tumors of the testis are benign, approximately 10% are associated with metastases. Malignant Sertoli cell tumors vary from slow to rapidly growing lesions, the latter exhibiting a considerable malignant potential. Two cases of malignant Sertoli cell tumor of the testis are reported. The literature is reviewed, revealing only six previously reported cases. The patients presented with testicular enlargement, which was present for 6 months and 8 years, respectively. There was no gynecomastia. The tumors metastasized to the inguinal and para‐aortic lymph nodes. The tumors were large, solid, but contained cystic spaces. No normal testicular tissue was visualized, and invasion of paratesticular structures was present. Microscopically, there was considerable variation in tissue pattern within each tumor, but tubular formations lined by cells resembling Sertoli cells were present, and the diagnostic importance of these features is emphasized.
