Summary:Community-acquired urinary tract infections occur in four major risk groups: in school-age girls, in young women in their sexually active years, in males with prostatic obstruction, and in the elderly. This review focuses on infections occurring in young women between late adolescence and the mid-forties. Six to seven million office visits annually in the United States are made because of acute uncomplicated urinary tract infections in this group, resulting in frequent time lost from work. 250,000 of these infections present as acute pyelonephritis, which often necessitates hospitalization and initial parenteral antimicrobial therapy. In 20% of women, frequent recurrences of infection follow the initial episode, producing additional morbidity and time lost from work. Both maternal and infant morbidity may occur in women who suffer infections during pregnancy.
