Summary: | Based on personal experience as US Surgeon General over the last seven years, the issues of health promotion & disease prevention are considered, with attention to questions of data development & to international health. These two current challenges for public health leadership in the US are also explored, focusing on changes in the public health landscape under the New Federalism of President Ronald Reagan. Although some perceive the public health system as in disarray, it is argued that the record does not support this view. Confusion between health care & medical care has arisen because those working in public health have not: focused on public health practice in a systematic way, identified the boundaries of the health care delivery system, or insisted on discrete & consistent definitions for practitioners within that system. Unlike the medical care system, there is no accreditation program for graduates of public health educational institutions, & there is no credentialing program for the Master's degree in Public Health.
