Summary:Tenosynovitis of the hand is a rare manifestation of extrapulmonary tuberculosis occurring in fewer than 5% of all cases of skeletal tuberculosis. In tuberculous tenosynovitis functional impairment remains minor for a long time, but the inflammatory process may spread progressively, leading to abscesses, rupture of the flexor or extensor tendons, and secondary infection of the bone and wrist. B-mode ultrasonography has shown great utility in evaluating soft tissue injuries of muscles and tendons. In the area of the hand, B-mode ultrasonography is used to evaluate different lesions, including liquid and solid masses and traumatic and inflammatory lesions, or to detect foreign bodies. Ultrasonography gives a reliable diagnosis in cases of tenosynovitis. Thickening of the tendon or synovial sheath and local accumulation of fluid are described as characteristic features for acute and chronic tenosynovitis and rheumatoid arthritis. We report on a patient with tuberculosis of the spine (Pott disease) and a concomitant tuberculous tenosynovitis of the hand. In this case ultrasonography has proved to be a useful imaging technique in the diagnosis of tenosynovitis.
