Summary:Tacrolimus is an immunosuppressive agent that can show a wide variety of neurological side effects, including leukoencephalopathy. The prognosis is usually good with complete neurological recovery after dose reduction or cessation. However, differentiation from progressive multifocal leukoencephalopathy can sometimes be difficult. We describe the case of a 52-year-old gentleman with severe, irreversible tacrolimus-related leukoencephalopathy 4 months after kidney transplantation.
