Two patients died. This report summarizes the preliminary findings of investigations in Nebraska and Maryland and implicated lot number 04015309 of Cobe Centrysystem 3 Blood Tubing sets (Gambro Healthcare, Lakewood, Colorado) as the cause of these reactions.
