Summary:A polymerase chain reaction (PCR) panel of CSF for SARS-CoV-2, herpes simplex 1 and 2, Haemophilus influenzae, Mycobacterium tuberculosis and Listeria monocytogenes, as well as CSF, blood and urine cultures were negative. The patient was diagnosed with seronegative NMOSD, on the basis of the presence of two core clinical characteristics of acute myelitis with longitudinally extensive transverse myelitis (LETM) and area postrema syndrome, dissemination in space (spinal cord, brainstem and cerebellum) and neuroimaging features associated with the core clinical findings (spinal cord and medulla lesions on MRI) (8). Viral infections are well known to induce an inflammatory response against the CNS, to activate myelin-specific T cells and by this way to accelerate the development of early or delayed virus-induced demyelination (16).
