Summary: | Anesthetic considerations for children with congenital cardiovascular disease undergoing corrective surgery are largely similar to those for adult patients with acquired cardiovascular diseases undergoing surgical procedures. There are, however, several controversial issues in anesthetic management that we believe are unique to pediatric patients because of age, size, disease process, or requirement for application of special techniques; 5 will be considered in this article. Broader and more detailed discussions of anesthetic considerations for pediatric cardiovascular patients may be found elsewhere. In this article we confine discussion to: 1) appropriate sites for arterial pressure monitoring; 2) management of hypercyanotic spells during anesthesia; 3) anesthesia requirements for the newborn; 4) assessment of adequate flow and perfusion pressure during cardiopulmonary bypass; and 5) mechanisms of neurologic damage after bypass, especially the syndrome of choreoathetosis. |
