The records were analysed according two dimensions, i. e. the content and the stucture. The proportion of notes describing nursing care increased in total with 83% after intervention According to the nursing process, most notes concerned descriptions of the patients needs/problems (83%) and evaluations of care given (9%) prior to the intervention. The same pattern was seen after the intervention was implemented. Descriptions of the patient's needs concerning sleep/rest and state of mind were most frequent before as well as after the intervention. Of interest was that there were no notes found describing spiritual needs and unexpected few notes describing care related to ADL and communication. |
