Summary:This chapter reviews the anatomy, indications, and technique for utilizing the palatal island flap in head and neck reconstruction. The unique anatomical features of this flap will be highlighted, in addition to practical tips in harvesting and insetting this flap. Potential pitfalls and complications will also be discussed. Strategies for use of this flap in specific subsites, and in combination with other reconstructive techniques, will be addressed. The palatal island flap remains a reliable and useful tool in the armamentarium of the reconstructive head and neck surgeon, to address challenging defects of the oral cavity and oropharynx.
