Summary:The purpose of this study was to evaluate the response to primary stent placement in elderly patients with ASRVD. Pre and post stent BP response and renal function together with selected other variables were assessed after one year of follow-up. An association was seen between renal function evolution and baseline creatinine (P=0.008). Of particular note, the highest creatinine group (>2.4 mg/dL) was more likely to improve. A negative correlation between renal function evolution and the Renal Resistance Index was noted. No improvement was seen with a resistance index >80%. Patients with higher values of baseline BP were more likely to improve, especially those with isolated systolic hypertension (53%) and systolic-diastolic hypertension (67%). Those with baseline creatinine >2.4 mg/dL were more likely to worsen. Those with hypertension refractory to 3 drugs showed improved BP in 84%. Those with a solitary functioning kidney were more likely to have improved BP (76%). Restenosis was seen in 26% of arteries (30/115) for the 90 patients evaluated after 12 to 18 months. When considering intervention in patients with ASRVD, careful consideration of baseline variables such as serum creatinine, severity of hypertension, type of stenosis and renal resistance index may help predict BP and renal function response.
