Introduction
The aim of this study was to evaluate the eventual presence of subtle clinical REM sleep-related motor signs in subjects without REM Sleep Behavior Disorder (RBD), by means of the RBD Screening Questionnaire (RBDSQ) and RBD Questionnaire - Hong Kong (RBDQ-HK) and to correlate them with REM sleep without atonia (RSWA), measured by the Atonia Index (AI).
Methods
In total, 243 subjects (mean age 57.7 years) were consecutively recruited, from a cohort of patients undergoing PSG for the possible presence of a sleep disorder; among these, 21 patients with concurrent RBD (mean age 69.9 years) were identified, who had average MMSE=25.5, AI=0.634, RBDSQ=8.1, and RBDQ-HK=17.3. After this, our analysis focused on subjects without RBD and >40 years of age (164 patients, 84 males and 80 females, mean age 62.6 years), of whom 38 (23.2%) had AI 0.8).
Conclusion
The independence of RSWA from the presence of other sleep disorders and the correction made for age and sex of the subjects, allows us to advance the hypothesis that with this approach we outlined a possible clinical picture of subjects with “subclinical RBD”. Whether they are at risk for the development of full clinical RBD or not needs to be established. Finally, the independence of RSWA from other sleep disorders allows us to exclude a significant recruitment bias and probably indicates the validity of these results also for non-clinical populations.
