Summary:AimTo implement the dosing and monitoring recommendations of the national gentamicin guideline for neonates1 and the British National Formulary (BNF) for Children.2 To assist the transition with the development of a decision-making flowchart.MethodA baseline audit (n=30) was conducted in the neonatal unit (NICU) to assess compliance with the national guideline and highlight areas to target for change and improvement. A new local gentamicin guideline and a decision-making flowchart were developed and implemented. Education on the new practices was provided. A post-implementation audit (n=28) was performed to assess compliance and measure changes.
