Summary: | Memorial Sloan Kettering Cancer Center is surveyed by The Joint Commission and the New York State Department of Health for proper tracking and documentation of tissue (ie, any human and nonhuman cellular-based products classified as tissue by state law, regulations, or the US Food and Drug Administration). Tissue tracking documentation starts from the moment the tissue arrives at the facility and follows it through every step to when it is implanted, discarded, or returned to the company. Complete and accurate information, such as the temperature on arrival, arrival time, person receiving it, and person retrieving it from storage, is essential for regulatory compliance, patient safety and billing, accreditation, and licensing. With the onset of the COVID-19 pandemic and an increase in vacancies at our facility because of staff member redeployment and an increase in staff member turnover, errors and discrepancies in TT documentation were observed. We wanted to be proactive and address this problem to avoid future potential errors. |
